General Passport Information - READ THIS FIRST!
Passport Basics:
- REQUIRED!! A check, cashier's check or money order for application fee (See Passport Fees section) payable to the 'US Dept of State'.
- DO NOT sign the passport application until requested to do so.
- An adult passport (16 years of age and older) is valid for 10 years.
- A child's passport (ages 0-15 years old) is valid for 5 years.
- If a name is different than the name on the birth certificate, you may be required to provide a legal document (such as a certified copy of a marriage license) to verify the name change.
